STEP 4: THE COMPLETED IEP:
The completed IEP will contain:
Once you have received the proposed IEP and placement, you have 30 days to respond.
You have three response options:
If you do not respond after 30 days, the plan is considered rejected. No part of the IEP can be implemented without written parent consent.
If you have any questions or concerns about what is written in the IEP, contact St. Agatha Resource Center for assistance.
Next, sign and return the IEP to your public school. Usually two copies of the IEP will be returned to you. Once you have the completed IEP, contact St. Agatha Resource Center in order that a copy will be on file at St. Agatha School.
Keep all your records of the IEP process in a safe place. (Refer to "Keep All Your Information Organized" in Parent Resources).

- The Team's vision for your child
- The child's present levels of academic achievement and functional performance
- Program and assessment accommodations
- Specially designed instruction and/or related services
- Measurable annual academic and functional goals, with benchmarks or short-term objectives
