STEP 1: PROCEDURE FOR REQUESTING TESTING
You can refer your child for an initial evaluation for special education whenever he/she is suspected of having a disability that may interfere with his/her learning.
This notice will include a list of the types of assessments they intent to complete and a request for consent. You may consent to all, some , or none of the recommended assessments, as well as request other assessments.
Keep the Resource Center updated and remember we are there to help you through the evaluation process and subsequent events.
You can refer your child for an initial evaluation for special education whenever he/she is suspected of having a disability that may interfere with his/her learning.
- Begin by contacting the public school your child would attend if not enrolled at St. Agatha. Put your request in writing and address the letter to the principal of your neighborhood public school.
- The letter should be brief and contain the following information:
- Start the letter by requesting special education testing.
- Provide your child's name, grade, date of birth, and include that your child attends St. Agatha School.
- List your concerns and any concerns that classroom teachers have expressed.
- If needed contact the Resource Center for assistance with the letter writing process.
- Although requests for testing are not required to be in writing, recoding your request provides you with a record of your concerns. Keep a copy of the letter.
